CiDMO Enrolment Form

The purpose of the CiDMO Enrolment Form is to obtain from you the information we need to enrol you into the New Zealand Certificate in Domestic Maritime Operations (Restricted Limits) (Level 4 ).

Accessing the CiDMO Enrolment Form

Completing the Form 

Complete the form digitally by opening it in Adobe Acrobat Reader and using the Fill & Sign option. If you do not have this application on your device, you can access it here Adobe Acrobat Reader.

Signing the Form 

Your CiDMO Enrolment Form must be signed. This can be done digitally, after completing all the required sections with the Fill & Sign option, using .

Sending the Form to Coastguard Boating Education

Once completed and signed, email the form to nzcidmo@boatingeducation.org.nz along with any additional documentation that is required for government funding purposes.
